Topic:  From Hospital to Home: The role of nurse-led multidisciplinary HF transitional care with insight from real-world Quality of Care indicators measurements

Chronic heart failure is associated with increased risk of recurrent hospitalisation, with associated risk of mortality and healthcare cost burden. Coordinated transitional care plan after an index heart failure hospitalisation may improve patients outcome and reduce the risk of early re-hospitalisation. Queensland Heart Foundation Support Services are comprised of nurses with specialist medical support. Some services include pharmacists, physiotherapists or exercise physiologists, and other allied health professionals. The Queensland Heart Failure Services Steering Committee oversees and supports a state-wide network of heart failure multidisciplinary support services in 24 sites, and have been in operation since 2006. In 2016, in partnership with the Queensland Cardiac Outcomes Registry (QCOR), we reported our inaugural annual report of patients characteristics and quality of care indicators among patients referred to the Queensland Heart Failure Services. Join us in exploring the management goals and barriers in optimising transitional care among patients with chronic heart failure.

Registration for this conference is online ONLY – secure your place straight away if paying by credit card. Credit cards accepted are MasterCard & Visa (please note AMEX and Diners are not accepted). A surcharge applies to all credit cards. 

If you are unable to pay by credit card, EFT transfers will be acceptable also. Please choose the “Pay by Direct Debit” option at the end of your online registration and use your tax invoice to process EFT payment. Full payment details will be stated on your tax invoice.

Payment not received within the indicated time frame will result in late payment surcharges and subsequently in cancellation of the registration in question. If your workplace is paying your registration, kindly make sure that they are aware of this. It is your responsibility to follow up that payment was processed within the indicated time frame and that a remittance advice was sent.


All cancellations or alterations must be put in writing to the ACNC 2018 Conference Secretariat.

If cancellation is received in writing:

  • By 9 January 2018 – $100.00 cancellation fee will apply
  • By 9 February 2018 – 50% of registration monies paid will be refunded
  • After 9 February 2018 – no refund on monies paid.

You may however transfer your registration to someone else at no extra cost.

Please note that any accommodation booking cancelled within 30 days of the conference is subject to 100% cancellation fees.
